Since 19 May, ongoing hostilities and precarious road access have prevented patients from reaching the hospital. On 21 May, the hospital’s third floor was reportedly hit, injuring a staff member. Hostilities in the area also damaged the hospital’s water tank and pipeline. On 22 May, the hospital’s third and fourth floors were reportedly hit, injuring two health workers. Patient tents, including one provided by WHO, caught fire, which also burned all medical supplies in the warehouse and destroyed vehicles in the basement. Al Awda faces an imminent risk of closure due to ongoing insecurity and restricted access, according to WHO.

On 18 and 19 October, Israeli strikes reportedly impacted all three remaining hospitals in the North Gaza governorate – Kamal Adwan, Indonesian and Al-Awda. According to MoH, the top floors and front yard of the Indonesian Hospital, which hosted more than 40 trauma and chronic patients as well as medical staff, were hit and had a total power outage. Two critical patients subsequently died amid lack of electricity and supplies, MoH added. Al-Awda Hospital and the entrance to the laboratory of Kamal Adwan were also hit, resulting in casualties, Al-Awda Health and Community Association and MoH reported. On 21 October, a complex mission led by the World Health Organization (WHO), together with OCHA, the United Nations Mine Action Service (UNMAS) and the Palestine Red Crescent Society (PRCS), managed to evacuate 14 critical patients, alongside 10 caregivers, from Kamal Adwan to Al Shifa Hospital in the Gaza governorate. The mission, which departed from Deir al Balah on 20 October, had to spend the night at the Kamal Adwan Hospital due to intense fighting in the area and was only able to proceed to Al Shifa the following day. Overall, as of 21 October, 100 in-patients remained hospitalized at Kamal Adwan, including seven adults and seven children in the Intensive Care Unit, with a constant influx of new casualties flooding the emergency department; some 200 trauma patients arrived in the space of 24 hours between 20 and 21 October, including 53 already deceased. On 21 October, the Director of Kamal Adwan reported that blood units had been exhausted, medical crews have been working restlessly without food, a number of patients had succumbed to their wounds amid capacity constraints, and other wounded persons were lying in the streets with no possibility to rescue them. Meanwhile, communication challenges caused by internet outages are hampering efforts to ascertain conditions at the Indonesian and Al Awda hospitals, informs the Health Cluster. Moreover, no UNRWA medical points are currently operational in North Gaza.

The Indonesian Emergency Medical Team (EMT), which had been deployed to the hospital by the World Health Organization (WHO) on 1 December following five access denials, was forced to evacuate as displaced people, caregivers, and many injured patients began fleeing the hospital and panic spread, according to the WHO Director-General, Tedros Adhanom Ghebreyesus. The WHO chief added that 33 people were killed just outside Kamal Adwan amid intense bombardment and hostilities near the hospital, which he described as one of the last lifelines for the people of North Gaza. Moreover, on four occasions between 2 and 8 December, the Indonesian Hospital in Beit Lahiya was attacked, resulting in the injury of three medical staff and six patients and damage to its electricity generator and water tanks. Also in North Gaza, the fifth floor and courtyard of Al Awda Hospital in Jabalya were reportedly hit on 3 December, with no casualties reported.

Access to critical health services continues to shrink as additional evacuation orders are issued and military operations intensify in areas where these services are found. On 14 May, Médecins Sans Frontières (MSF) announced that it had withdrawn from the Indonesian Field Hospital after providing post-operative care for injured patients at the facility since December 2023. The NGO reported taking this decision following the “advancing offensive” in Rafah and the “pattern of systematic attacks against medical facilities and civilian infrastructure” it had witnessed since the beginning of the conflict, adding that its teams have endured 26 violent incidents since October 2023, including “airstrikes damaging hospitals, tanks being fired at agreed deconflicted shelters, ground offensives into medical centres, and convoys fired upon.” While downsizing its footprint in Rafah, MSF has now resumed operations at the Nasser Medical Complex in Khan Younis, providing orthopaedical surgery and care for burns, among other services.

On 2 November, the MoH reported that the main electricity generator of the Indonesian Hospital, in northern Gaza, had ceased to operate due to lack of fuel. This hospital has been receiving hundreds of people injured during the recent hostilities in Jabalia camp. Exposing hundreds of patients with serious injuries to imminent risk of death or lifelong disabilities. Shifa Hospital in Gaza city is reportedly almost out of fuel. Since the start of hostilities, 14 out of 35 hospitals with inpatient capacities are not functioning and 51 (71 per cent) of all primary care facilities across Gaza (72) are not functioning due to damage or lack of fuel.
